In the variables near the top of the link, you'll notice the "text" variable has been replaced with an image. The "link" variable has been replaced with a "spacer" image to act as the button the potential respondent can click on to access the survey.
 
In the source URL below the variables (the second set of script tags within the example), you'll notice various items appended onto the end of the survey URL. We can append a few more fields onto the end of the URL to further edit the embedded popup.

This is useful when posting a survey on various pages of a website, in order to know which site they came from.

In order to track the URL the survey taker came from when starting the survey...
*Paste your own survey link in place of the existing link in the example below ''("http" through "SVID=Prod")''.
*Replace "Click here to take survey" with the text you would like to appear as the hyperlink.
*Use the code provided below (now edited with your link and text) as the hyperlink code you paste into your web site (in place of using the "a href" html for a regular hyperlink code).

In order to see the URL later, you will need to insert "URL" as an embedded data field in the Survey Flow. *Go to Edit Survey -> Survey Flow.
*Click on "Insert a New Element Here" at the bottom of the window
*Select "Embedded Data".
*Within the text box on the Embedded Data element, type "URL".
**This tells Qualtrics to display a column or field in the raw data and reports for the URLs it captures for each respondent.
