Submitting Answers with Table of Contents

For our mentor program, we have monthly checklists of items that need to be covered by the mentor with the mentee. Rather than creating an individual survey for each month, I'm utilizing blocks and the table of contents feature to show each month, plus mid-year and end-of-year surveys. The problem I'm running into is that we would like to track what mentors have completed their checklists for each month, but there doesn't appear to be a way to submit each month's completed checklist. Looking for advice/suggestions. Thank you!
