Allow user to "add" extra text entry box?

Hi there,

I am trying to build a question that allows the user to add an extra text field if he/she has more than one text based answer for the question.

For example:

"Please write next to the relevant confidence level, ’the reason why’ the customer needed service #2? You can add as many 'triggers or reasons’ as you like, simply press the ‘ + ‘ symbol to create another entry field.

1. Confident and assured:

+ The customer needed to get their drivers licence updated
+ The customer can see that our Smart Payroll service is down, and wanted to be advised when it will be up and running again."


And so on and so forth. These need to be text entry (not multiple choice) as it could be a whole host of different things, but needs to allow the user to add between 1 and 10 answers.

Does anyone have any idea on how to achieve this?

Hi @joshmutsch
You can use "In page" option in Add Display Logic. You can keep answer option as reason 1, reason 2 .... reason 10. On click of each option you can show a text box dynamically.
