How can I set my creative in order to close it when users click on "Send" and fill the survey?

Hi all,

I have a survey with a slider creative (see attachment below), user see the button "Aiutaci a migliorare", if they click on it they find and NPS with a comment box. I want to set this survey in order to close it, and hide the "aiutaci a migliorare" button when users click on "Send" and fill the survey.
If I should be able to do it using Javascrip.
Could you help me?
