Sounds like one of the previous Action Sets is being executed. Only one Action Set out of multiple will be executed, and Action Sets are evaluated in sequential order. So, most likely the conditions are being met for one of the Actions Sets above your last one. Since those conditions are met, before those of your last Action Set, the previous Action Set is executed and the last one never gets evaluated.

_Remedy:_ I would check the conditions of each of your Action Sets. Think of it like Branching in the Survey Flow of the Research Core. If you have 'Always' for any of the Action Sets above the last, click the 'Change' button next to the word Action and implement a set of conditions for each of the Action Sets.
