Include Display-Logic and Validation in a Side-by-Side-Questions

Hi!
I'm trying to implement a display-logic and validation on columns in a side-by-side-question.
When a participant doesn't check the middle option (neutral) but one of the other four options in the first column, the second and third column should appear. And a validation should happen that the particpant picked one of the options provided in the dropdown-list (not the standard one = "keine Wirkung")
I know there might be better options with different question types, but the side-by-side works best for the questions we'd like to ask.
I guess this might only be possible with a custom Javascript? Thanks for any help!
