About Payment, Billing, & Renewals

This page contains information to help you manage your financial interactions with Qualtrics. Here, you will find instructions for how to set up Qualtrics as a vendor, update billing contacts, send payments, and more.

If you need to submit a request or have additional questions, follow the instructions in Contacting the Account Services Team to reach out to the Account Services team directly.

Setting Up Qualtrics as a Vendor

Qtip: This is normally completed by a purchasing department or buyer.

If you need to set up Qualtrics as a vendor in your system to buy a license or software from Qualtrics, you can use the information below, which differs based on the entity you are contracting with. Entity information can be found within your contract.

Vendor Forms and Documents

The following vendor forms are available for Qualtrics. They may be needed when you set up Qualtrics as a vendor. Click the name of a file to download it.

  • Qualtrics LLC W-9: 2024 W-9 tax form. Next year’s form is typically available by February each year.
    Qtip: This file is applicable when setting up Qualtrics in the United States.
    Qtip: If the tax ID on your W-9 is different from the one on your invoice, see the Qualtrics 2024 W9 Cover Letter.
  • Certificate of Liability Insurance: Certificate of liability insurance. Next year’s form is typically available by July each year.
  • Tax Residency Certificate: Certificate of residency. Next year’s form is typically available by May each year.
  • Certificate of Formation: Document validating legal operation for Qualtrics, LLC.
  • Certificate of Existence: Document validating legal operation for Qualtrics, LLC as a business in the state of Utah.
  • Provo City Licensing: Document validating legal operation for Qualtrics, LLC as a business in Provo, UT.
  • DEI Letter: Qualtrics’ Representation of the organization’s demographic data.
  • Conflict of Interest Letter: Document confirming policies that prohibit Qualtrics officers and employees from holding a financial interest in customers, if such interest could lead to a conflict of interest.
  • Qualtrics Ireland Tax Clearance CertificateTax clearance certificate for Qualtrics Ireland.
  • Qualtrics LLC 10F: 2024 10F tax form. Next year’s form is typically available by May each year.

Viewing Invoices

Invoices are generally sent 30 days before your annual billing date, to the billing contact on file for your organization. See Customer Contact Information for more information on billing contacts.

Your contract also has estimated invoice dates within the fees exhibit. If you haven’t received an expected invoice, follow the instructions in Contacting the Account Services Team to submit an inquiry.

Qtip: To get a copy of the contract associated with your invoice, submit a request to the Account Services Team.

Updating Customer Contact Information

To update the name or address of your organization, submit an inquiry with the Account Services Team.

Billing Contact

You can change the billing contact for your organization within the Billing Details section of the Customer Success Hub if you are a Brand Administrator, or listed as the primary or billing contact for your organization. For step-by-step instructions, see the “Primary and Billing Contacts” section of the Billing Details support page. If you are not a Brand Administrator or the primary or billing contact, use this survey to submit an inquiry.

Qtip: To change the billing address or billing details for your organization, Brand Administrators, primary contacts, and billing contacts can submit a request to Account Services. See Contacting the Account Services Team for more information.

Collections Communications

Aged invoices are collected by ALTUS Receivables Management, a third party collections agency employed by Qualtrics to collect on aged invoices. If an invoice has been sent to ALTUS Receivables Management, the payment obligation to Qualtrics is now being collected by this third party collections agency. Please work with ALTUS directly to settle your outstanding balance.

Collections communications are sent to the billing contact email address on file. If this email address is incorrect, please update it by following the instructions in the “Primary and Billing Contacts” section of Billing Details. If you are not a Brand Administrator or the primary or billing contact, use this survey to submit an inquiry.

ACH Payments

Attention: ACH payments are only available for customers in the US and Europe.

Automated Clearing House (ACH) payments are electronic transfers of funds between bank accounts. ACH payments allow you to submit a payment to Qualtrics using your bank information.

When you submit an ACH payment to Qualtrics, your bank account is verified. Verifying bank accounts helps protect transactions by confirming the identity of the account holder. The account verification process consists of the following steps:

  1. Provide account information: You provide your bank information to Qualtrics. This may include your account number, routing number, and more.
  2. Receive micro-deposits: 2 small deposits (normally under $1 each) are made to your account.
  3. Confirm deposit amounts: Confirm the exact amounts of the deposits you received.
    Qtip: You will receive an email from Stripe asking you to confirm the amounts.
  4. Activate account: Once your account has been verified, it can be used for ACH payments.

Contacting the Account Services Team

If you have questions about payment, billing, or renewal inquiries not covered on this page, you can reach out to Account Services for support.

Inquiries are typically responded to within 3 business days. If you haven’t received a response within this time frame, please follow up on your case or email.

Attention: This option is only available to Brand Administrators, primary contacts, and billing contacts. If these roles don’t apply to you, use this survey to submit a payment, billing, or renewal inquiry.

When to Contact the Account Services Team

Below is a list of scenarios that require you to submit an inquiry to the Account Services team. This does not cover all possible scenarios; if you have a question that isn’t covered in the following list or on this page, submit an inquiry.

  • Requesting a refund
  • Escalating a payment, billing, or renewal issue
  • Fixing an error or incorrect tax amount on your invoice
  • Providing your tax exemption form
  • Providing your Purchase Order information
  • Getting your invoice uploaded to your payment portal
  • Having Qualtrics register for your payment portal
  • Discussing a payment plan
  • Confirming a payment
  • Disputing an amount due
  • Notifying Qualtrics you are in bankruptcy
  • Avoiding being suspended for non-payment
  • Requesting to stop receiving emails trying to collect on your payment
  • Updating your billing address or billing instructions
